Norfloxacin IP 800 mg Description Norfloxacin is a synthetic broad spectrum antibacterial agent belonging to the fluoroquinolone group. It exerts its bactericidal effect by inhibiting the A subunit of DNA gyrase, an essential enzyme involved in DNA replication. Indications URINARY TRACT INFECTIONS Acute, uncomplicated, complicated, chronic and recurrent UTI. BACTERIAL ENTERIC INFECTIONS Bacterial gastroenteritis caused by enteropatho-genic E.coli and Shigella sp. SEXUALLY TRANSMITTED DISEASES Gonorrhoea, gonococcal urethritis and cervicitis, and chancroid. Dosage and Administration Norflox should be taken one hour before or two hours after meals with a glass of water. Patients should be advised to drink fluids liberally. Antacids should not be taken concomitantly or within two hours of dosing. URINARY TRACT INFECTIONS Uncomplicated 400 mg twice daily for three to ten days Complicated 400 mg twice daily for up to 21 days. Acute gastroenteritis 400 mg twice daily for five days. Gonococcal urethritis or cervicitis A single dose of 800 mg. In impaired renal function If creatinine clearance is between 10 and 30 ml/min the dose should be reduced to 200 mg twice daily or 400 mg once daily.
Contraindications Hypersensitivity to the quinolone group of antibacterials. Warnings and Precautions Probenecid: When co administered with norfloxacin, probenecid diminishes the urinary excretion of norfloxacin. Nitrofurantoin: Nitrofurantoin antagonizes the antibacterial effect of norfloxacin. Theophylline: Plasma levels of theophylline have been reported to be elevated when norfloxacin is given concurrently. Oral anticoagulants: Norfloxacin, like other quinolones, may increase the effect of oral anticoagulants. Multivitamins: Multivitamins and preparations containing iron or zinc, antacid or sucralfate should not be taken concomitantly or within two hours after norfloxacin. PREGNANCY Should be used during pregnancy only if the potential benefit justifies the potential risk to the foetus. NURSING MOTHERS A decision should be made to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother. PAEDIATRIC USE As with other drugs of this class, ciprofloxacin has been shown to cause arthropathy in weight-bearing joints of immature animals. Hence ciprofloxacin is usually not recommended for use in children. PAEDIATRIC USE Fluoroquinolones have been shown to cause degenerative changes (arthropathy) in weight bearing joints of young animals. These agents should preferably not be used in children. Crystalluria: Inadequate intake of water when on norfloxacin can cause crystalluria. CNS stimulation: Norfloxacin should be given with extreme care to patients with a history of seizures, and severe cerebrovascular disease. Side Effects Some cases of nausea, headache, dizziness, fatigue, rash, abdominal pain, dyspepsia, somnolence, depression, insomnia, constipation, flatulence and heartburn have been reported. Less frequent reactions include dry mouth, diarrhoea, fever, vomiting and erythema.
